Our office is an outpatient clinic and not an urgent care center. We cannot see patients who are in a crisis and the patient is requested to call 911 or go to the nearest emergency room in these situations. If you are an existing patient and in a crisis, please follow the above procedure. You can contact Dr Pol at the main office tel. No and follow directions if the call is after business hours. Most patients in a crisis will need evaluation in the emergency room. Patients may also consider calling the Georgia Crisis Center at 1-800-715-4225.
We have been providing telemedicine appointments since March 2020.
Please note the following:
The patient must be present at the time of the appointment ( especially in the case of children under age of 18 yrs) (similarly for young adults, their parents cannot be a substitute for the patient at the time of the appointment).
On the day of your appointment you will receive a text message and email with a link for the telemed appt. Click on that link 5 minutes prior to your appointment. You may need to open the link on your computer in case you are unable to do so on your cell phone.
Please ensure that you allow camera and microphone access on your device ( cell phone or laptop computer). Please ensure you are seated for the appointment and avoid moving around from one room to another. Please do not drive during the call. If the connection is weak then Dr. Pol will call you via another app e.g Doximity and will complete the appointment . In case the call is disconnected or the video is ‘frozen’, Dr. Pol will call you back on the phone.
You can call the main office tel no and talk to staff or leave a message that will be conveyed to Dr. Pol during regular business hours. For after hours contact you can call Dr. Pol on his cell phone (only for urgent situations/emergencies), listed on the office voicemail , In case of emergencies you are advised to call 911 or go to the nearest emergency room.
If we are a provider on your Ins. plan the visit will be covered by the Ins plan and you will be financially responsible for the amount due (deductible, copays, co -insurance) from you per the contracted rate with the Ins co. based on the CPT coding for the visit.
Please note that ultimately the patient/guarantor is ultimately responsible for ensuring that payment is provided. In case of children/adolescents-payment is to be done by the parent present at time of appointment ( especially relevant in case of divorced/separated parents or blended families)
Payment is expected at the time of the appointment, including any previous balance due. In case of telemedicine appointments, the staff will collect payment over the phone (preferred ), prior to the appointment or immediately after the appointment. Your payment will depend on the contracted fee with your insurance company and the CPT code used for that visit.
Payment is expected at the time of service (for telemed appts, payment is collected over the phone). We will send bills by email or mail if needed and there is an extra fee of $10 for the same.
Payment amounts are based on the CPT Code use for that visit / service provided. CPT codes are based on complexity of the visit and the fee is based on the contracted rates with your Insurance company. These rates vary for each patient and for different Insurance companies.
Note : The above also applies for SELF PAY patients, payment is based on the CPT code used. We do not use a flat fee for self pay services. Non Payment- can lead to termination of treatment with appropriate notice.
We accept cash and credit cards ( Visa, Mastercard, Discover). We do not accept American Express.
We require 48 business hours notice to cancel/reschedule appointments. If not then, there will be a $50 charge for late cancellation /no show fee. If appointments are missed frequently, then treatment may be terminated and a recommendation will be made to continue treatment with another provider. Non compliance with treatment impacts the patient’s overall progress for the patient.
This means your Insurance company will not approve / pay for your medication without the provider completing paperwork justifying the medication is needed. Many times Insurance companies will not approve ‘Brand’ medication unless the patient has tried or failed generic medications in that category. In some instances Brand medications may not be on your Insurance plan formulary.
We charge the patient an administrative fee of $ 20 for completion of prior authorization forms per medication and there is no guarantee that despite completion of such forms that the medication will be approved by the Insurance company.
These are medications typically used for treatment of sleep disorders/ anxiety conditions and ADHD. They have a potential for addiction/dependency and therefore are strictly regulated by the DEA. The DEA monitors the patients, pharmacies and physicians and as a provider, the doctor has to check the Georgia Prescription monitoring database for each patient on a controlled medication to ensure appropriate use of these medications.
In some cases testing may be needed for diagnostic clarification or to evaluate for Learning disorders. In many cases with complaints of adult ADHD symptoms, testing is needed to rule out any other disorders.
